POSITION SUMMARY
We are seeking a dedicated and compassionate Family Medicine Physician to join our team. The Family Medicine Physician role is based on expected duties for any primary care physician in an ambulatory setting. At Polara Health Medical Group (PHMG), this role will be an integrated role with the behavioral health team including other physicians, advanced practice providers, therapists, etc. The expectations include, but are not limited to, managing a panel of patients and productivity that meets the standards set by PHMG. A focus on the triple aim of improved patient outcomes, superb patient experience, and efficient care will be the key to success at PHMG.
ESSENTIAL RESPONSIBILITIES
- Managing a patient panel with a broad array of patient needs including acute issues, chronic diseases, and wellness promotion. Chronic disease management will be a hallmark set of duties including medication management.
- Diagnosing and managing adult patients using electronic tools within the EHR, as well as personal communications putting the patient first always.
- Documenting complete notes on a timely basis assuring the proper patient information is available and useful for other team members in transitions of care, according to Polara documentation policy.
- Help train and support clinic staff through a team-based model. Lead the care team through influence to assure a holistic patient experience.
- Appropriately lead and coordinate care with other team members, specialists, and outside agencies/health entities to assure that the patient’s care is optimized.
- Always be willing to seek and accept constructive feedback, as well as mentor and support new providers who join the team.
- Review and utilize quality, experience, operational, and efficiency data in daily practice to assure value (quality/cost) organizational goals are met.
- Participate in team meetings and other committees as requested and appropriate (citizenship).
- Support leadership decisions, while at the same time be forthright in bringing for the feedback and ideas for improvement.
- Maintain Polara Health Medical Staff membership.
QUALIFICATIONS
- Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
- Graduation from an accredited medical school recognized by the Arizona medical boards (MD or DO). Completion of an accredited residency in Internal Medicine or Family Medicine.
- Board certified by the American Board of Internal Medicine or the American Board of Family Medicine.
- Active Arizona license to practice medicine.
- Active DEA certificate.
- Eligible for Polara Health Medical Staff membership.
- Two years of prior practice experience preferred.
- Experience with behavioral health a plus.
